On your first visit to the Health Centre you will be asked to complete:

Client Registration and Health Assessment form
As well you will be asked to read information on Your Personal Health Information and our Practices and sign the Client’s Statement of Understanding.

The Health Centre is committed to protecting the privacy and confidentiality of your personal health information. Medical information will be released only with your written consent. There may be some situations where we may be required by law to disclose your personal health information without your consent.

These forms may be completed in person at the Health Centre OR: to save time, you can DOWNLOAD the forms and bring the completed forms with you when you come to the Health Centre for your first visit.
