Health Centre Registration

On your first visit to the Health Centre you will be asked to complete:

• Client Registration and Health Assessment form
• Well Female Exam form (for female students coming to see us for birth control and/or pap smear/well woman exam).
• As well you will be asked to read information on Your Personal Health Information and our Practices and sign the Client’s Statement of Understanding.

These forms may be completed in person at the Health Centre OR: to save time, you can download the forms and bring the completed forms with you when you come to the Health Centre for your first visit.
