The health centre is committed to protecting the privacy and confidentiality of your personal health information. Medical information will be released only with your written consent. There may be some situations where we may be required by law to disclose your personal health information without your consent (please read Your Personal Health Information and our Practices for more information).

On your first visit to the Health Centre you will be asked to read:

• Your Personal Health Information and our Practices and to sign the Client’s Statement of Understanding.
• To expedite your visit, we invite you to download these documents and bring the completed form to the office on your first visit.
