Case Coordination and Service Navigation

Students engage in case coordination and problem solving to support service navigation, writing skills to advocate for transition, funding, referral, and service continuity for individuals with disabilities, birth to emerging adulthood and their families. Students engage in advanced professional writing to facilitate case coordination and service navigation within a variety of systems. Students apply person/family-centred practice and responsive inclusive planning through accessing and navigating multiple services and sectors in order to support a continuum of service for individuals/families with disabilities and complex needs.
